Comment | Functional Safety management Plan (FSMP). FSMP shall be established addressing all activities of the SIS lifecycle and detailing each phase due to be performed (e.g. project development or operation phase). This shall include a plan for when the activities shall be performed and responsibilities for the different activities. The document shall be made available by the overall SIS compliance responsible, i.e., integrator. SIL Allocation Report. With references in analysis of hazards, risks and risks reduction, the determined Safety Instrumented Functions (SIF), allocated Safety Integrity Levels and identified SIS respectively shall be documented in a "SIL allocation report" file including an index of all SIF/SIS. LOPA Report. Layer Of Protection Analysis (LOPA) is a simplified risk-based method that can be applied to allocate SIL requirement to the Safety Instrumented Functions (SIF). LOPA is the preferred method for the SIL allocation of preventive Safety Instrumented Functions. LOPA analysis shall be documented in a report. Safety Requirements Specification (SRS). Safety Instrumented System (SIS) is developed throughout different lifecycle phases providing an extract of relevant information. The SRS shall focus on main requirements and performance parameters of the SIS while references are made to other documents in order to accomplish a complete overview of all SIS properties. SIL Compliance Report. This report including Calculation and Data Dossier (CDD), shall describe methodologies, all data sources applied, calculations and reporting the status of each SIS conformance with main requirements. Functional Safety Assessment reports. FSA shall be planned, performed and reported at the relevant project stages as indicated by IEC 61511. Miscellaneous SIS specific information, examples:
Formats for calculation files: Reports: DOC+ PDF. Calculation files (LOPA, SRS, SIL compliance including CDD): Excel (+TBA if commercial software applied) |
NORSOK Z-001 rationale | Documentation shall fulfil the requirements in the standards IEC 61511 and IEC 61508 or Norwegian Oil and Gas guideline 070 as relevant and be developed to suit any lifecycle of a Safety Instrumented System (SIS). |
